Patellofemoral Pain Syndrome

What is Patellofemoral Syndrome?

Pain around the kneecap is often referred to as patellofemoral pain syndrome. PFPS is caused by mal-tracking of the knee cap, which should run straight up and down in the groove at the front of the knee when the knee is bent and straightened. Sometimes the knee cap will move too far to the outside of the joint, resulting in pain.

The injury is common among girls aged 10 - 20 years. Correct rehabilitation is essential for this injury along with taping. Do not ignore this injury because if it gets very bad it is extremely difficult to treat.

Chondromalacia patella is a separate condition which may be due to PFPS. CMP is damage to the cartilage which covers the back surface of the knee cap. Mal-tracking may cause this, where the underside rubs against the bone underneath. However it may also be caused by an impact on the kneecap.

Symptoms of Patello femoral syndrome:

  • Aching in the knee joint, particularly under the patella, around the patella and at the front.
  • Straightening and bending the knee may cause pain and clicking
  • There is tenderness along the inside border of the kneecap.
  • Walking up or down hills or stairs makes it worse.
  • Sitting for long periods may be uncomfortable. This is known as the theatre sign.

Who is at risk?

  • If your kneecap sticks out or is small.
  • If your feet roll in or pronate.
  • Weak quadriceps.
  • Runners who participate in a lot of long distance running or hill running.
  • Those who have had a previous knee dislocation

Treatment of patellofemoral syndrome:

  • Apply RICE (Rest, ice compression and elevation).
  • Rest until there is no pain (this is very important).
  • Use a knee support or heat retainer (with a hole for patella support) or tape the knee.
  • See a sports injury professional who can advise on treatment and rehabilitation.
  • Rehabilitation to correct muscle imbalances such as weak VMO and glutes.
  • Provide orthotic foot supports.
